Nobody has ever captured MDMA inside its real target in the brain. One picture exists of it held by anything at all — an antibody, solved in 2009 — and that is what gets measured here.2

This is a therapeutic antibody raised against methamphetamine — not the serotonin transporter. It is shown because it is the only experimental structure with MDMA bound. No crystal or cryo-EM structure of MDMA in SERT exists. Every published image of that complex is a computational docking model, usually built into paroxetine-bound4 or serotonin-bound5 SERT.

The structure exists because someone was building a treatment. The protein around the molecule is a single-chain antibody fragment developed as a candidate therapeutic against methamphetamine — the idea being to bind the drug and hold it before it reaches the brain. It was crystallised with two substrates to see how it grips them, and MDMA was the second.3

So what you are looking at is a molecule held in something designed to capture it, rather than in the transporter it acts on. The pocket is not SERT and the geometry is not SERT's. What it does show, measured rather than modelled, is the molecule's own conformation — which ring sits where, and how the amine presents.

That gap is worth stating plainly, because it is the interesting part: one of the most studied recreational compounds of the last forty years has never had its complex with its primary target solved experimentally.

Mechanism

Substrate, not blocker.

The common antidepressants — SSRIs — plug the transporter and stop it working. Serotonin builds up outside the cell because it cannot get back in. The pump is blocked, but it is still pointing the same way.

MDMA does something else. The transporter picks it up as if it were serotonin and carries it inside — and in doing so it starts running the other way. Serotonin gets pushed out of the cell instead of pulled in.6 The pump does not stop. It runs backwards.

That is the difference worth knowing, and it is why this class of drug is named after what it does rather than what it binds to.

The molecule

It comes in two mirror-image forms. Only one of them has ever been photographed.

MDMA comes in two mirror-image forms — like a left and a right hand, same parts, not interchangeable. The one called S carries the stimulant and empathy-like effect; the one called R leans more hallucinogenic.8

The ligand in 3GM0 is the S enantiomer — the half the word empathogen refers to.2 The trials used both halves mixed together, which is how the compound is normally made.7

So the only form anyone has managed to capture is exactly the half this token is named after, while the thing actually tested on patients was the mixture.

The record

Where the clinical research actually stands.

MDMA-assisted therapy is not an approved medicine. It has been through phase 3, been refused, and been resubmitted. That is the whole position, and it is worth stating without varnish.

Status, plainly

Not approved by the FDA. In the United States the only lawful routes of access are enrolment in a clinical trial and FDA Expanded Access for individual patients.9 Nothing on this page is medical advice, and this project has no connection to any trial, sponsor or institution named above.
